Western Cape launches pilot program to reward wellness in South Africa's public healthcare system

The Western Cape Department of Health and Wellness has initiated the first public healthcare reward program in South Africa, implementing a behavior-based wellness model for public sector patients.

This initiative, named WOWRewards, began as a long-term pilot project in the Westridge and Mitchells Plain areas. The program offers small financial incentives to residents to help them establish sustainable habits before chronic diseases develop.

The Minister of Health and Wellness for the Western Cape, Mireille Wenger, emphasized the need to bridge the gap between private and public healthcare offerings. She noted that while reward programs are familiar to many private medical scheme members, most South Africans utilize public healthcare services.

Wenger stated: 'Prevention is better than cure, but choosing healthy options is not always easy. With WOWRewards, we want to give people an extra incentive to take steps that can have a long-term impact on their health.'

Key features of the program and participation requirements

The program is available to residents of Mitchells Plain who use public healthcare facilities and are over 18 years old. It is planned to expand to Knysna in early 2027. The goal of this project is to enroll up to 2000 active participants across two locations to test the system's functionality in both urban and rural settings.

Participants can receive rewards of up to 100 rand per month for completing approved activities relevant to their circumstances. These activities include: registering for antenatal care before 20 weeks of pregnancy and attending scheduled visits; participating in approved community fitness classes and recognized parkrun events; weight monitoring; timely routine immunization of children; timely collection of chronic medication packs; and completing short educational modules on pregnancy and newborn care.

After activity verification, the corresponding reward will be issued via the Zlto digital platform. WOWRewards relies on the Western Cape on Wellness (WOW) community network, collaborating with Zlto and local partners to assist residents with registration and participation.

Long-term vision for public health

Department representatives view this initiative as a significant step toward reducing the long-term burden on the public healthcare system by fostering preventive habits. Dr. Sadiq Karim, Chief Operating Officer of the Provincial Department of Health and Wellness, pointed to the scientific basis of this strategy: 'Studies have shown that regular physical exercise combined with healthier dietary habits significantly reduces the prevalence of chronic diseases in people, improves mental well-being, and aids in weight loss—all of which substantially enhances quality of life.'

Provincial health leadership will monitor participant engagement throughout the implementation phase to determine if the program successfully supports behavioral change and warrants wider rollout across the province. Wenger highlighted the adaptive nature of the trial, noting: 'Since this is a pilot initiative, we will adapt and learn as it progresses. Our long-term goal is to assess how effectively these incentives motivate people to adopt and maintain a healthier lifestyle.'
