Chronic Stress and Its Impact on Metabolic Health and Cardiovascular System

Stress does not always remain in the realm of mental health; when it becomes a constant element of daily life, it can affect sleep quality, eating habits, as well as the processes regulating blood sugar levels and fat accumulation in the body.

This has significant consequences for residents of South Africa, as chronic stress is increasingly linked to metabolic health problems such as obesity, type 2 diabetes, and cardiovascular diseases. This link causes particular concern in a country that is already facing a serious burden of metabolic diseases, raising the question of the need to include stress management in long-term heart health protection programs.

According to data from the International Diabetes Federation, approximately 2.32 million adults in South Africa lived with diabetes in 2024, representing a prevalence of 7.2% among the adult population. Furthermore, the problem of stress is widespread in workplaces. A 2026 Gallup report on the state of the global workplace found that 36% of South African employees experienced significant pre-existing stress.

For people facing financial uncertainty, demanding workloads, and employment instability, prolonged stress can become not only an emotional burden.

Stress is a natural reaction of the body to pressure or perceived danger. The problem arises when this reaction remains activated for a prolonged period. One of the hormones involved in the stress response is cortisol. With chronic stress, prolonged changes in cortisol levels and other stress response systems can disrupt processes related to glucose regulation, insulin sensitivity, and fat metabolism.

Nicole Jennings, a representative of Pharma Dynamics, noted: 'Most people understand that stress can affect mood and sleep, but few realize that chronic stress can also disrupt the body's metabolism.' Studies have established a link between prolonged psychological tension and changes in glucose metabolism, reduced insulin sensitivity, and visceral fat accumulation in some individuals.

Visceral fat accumulates around internal organs and is metabolically active, so its excess is associated with an increased risk of conditions such as type 2 diabetes and cardiovascular diseases. However, the relationship between stress and metabolic health is not limited to biological aspects.

When people are under constant pressure, they may also change their eating habits, exercise less, or sleep poorly. Professor Naidu, CEO of the South African Heart and Vascular Foundation, asserts that the cumulative effect of daily stressors can affect many aspects of health, including nutrition, physical activity, and sleep.

One example is stress affecting nutrition. A person experiencing prolonged stress may turn to food for comfort or convenience, which over time can lead to weight gain. Excess weight and obesity, in turn, increase the risk of diseases such as high blood pressure and type 2 diabetes.

This creates a complex interplay between psychological stress and metabolic health. Jennings explained: 'Long-term emotional or work-related stress can increase the likelihood of developing these conditions, while metabolic disorders, in turn, can alter the body's stress response systems, potentially increasing vulnerability to stress consequences.'

In other words, stress can contribute to behavioral and biological changes affecting metabolic health, while living with a chronic illness can become an additional source of physical and emotional strain.

Sleep can also become part of this cycle. Poor sleep is both a frequent consequence of stress and a recognized risk factor for poor metabolic health. When a person constantly sleeps poorly, they may lack the energy for exercise, making it harder for them to maintain a healthy diet. The result is a cycle where stress affects sleep and behavior, these changes affect metabolic health, and deteriorating health generates additional stress.

The problem extends beyond diabetes and weight. Metabolic conditions are often accompanied by cardiovascular risk factors such as high blood pressure, abnormal cholesterol levels, and excess abdominal fat. Together, these factors increase the risk of heart disease and stroke.

This makes the link between stress and metabolism particularly relevant during Heart Awareness Month, when attention is focused on factors affecting long-term cardiovascular health. Professor Naidu emphasizes that the consequences of cumulative stress on health are not always immediately obvious, but they can accumulate over time.

There is also growing interest in the relationship between diet, gut, and brain. 'Gut-brain' axis research suggests that what people eat can influence aspects of mental well-being, adding another layer to the relationship between lifestyle, stress, and health.

Workplaces are another important topic for discussion. The South African workforce faces a range of pressures, from financial strain and rising cost of living to job instability and excessive workload. The Profmed Stress Index for 2025 showed that financial pressure and work-related issues remain significant sources of stress among South African professionals.

The survey also revealed the impact of stress on daily habits, including sleep patterns, diet, and physical activity. This means that workplace wellness programs cannot focus solely on employees' emotional state. Creating opportunities for movement, promoting healthier work methods, and ensuring employees have access to appropriate mental health support can be part of a broader approach to preventing chronic diseases.

The link between stress and metabolism does not mean that stress itself causes diabetes, obesity, or heart disease. Genetics, diet, physical activity, sleep, existing medical conditions, and other environmental factors all play a role. Nevertheless, reducing chronic stress can be part of a broader plan for health protection.

Regular physical exercise, adequate sleep, a balanced diet, quitting smoking, and moderate alcohol consumption can support overall health improvement. Mindfulness practices and strong social support networks can also help people cope with stress. Jennings believes that stress management should be viewed not as a luxury, but as part of caring for long-term health.

Professor Naidu urges people experiencing unmanageable stress to seek professional medical help, as trained specialists can assist with stress and mental health disorder management. The conclusion is not that every stressful day puts a person at risk of developing diabetes or heart disease. Rather, constant stress must be taken seriously as one component of the overall picture of metabolic and cardiovascular health. Paying attention to how stress affects sleep, diet, movement, and overall well-being can thus become an important part of long-term health protection.
