HealthSnap has secured $25 million in funding, which will be used to strengthen its financial position and develop its artificial intelligence-based virtual care platform. These funds will be utilized for investments in the AI platform, as well as for developing commercial initiatives and customer support programs nationwide.
The acquired investments will go towards product development, stimulating commercial growth, and expanding offerings in advanced primary healthcare. HealthSnap plans broader implementation of its solutions within leading U.S. healthcare systems, thereby enhancing the use of virtual care across the country.
Eastward Capital Partners led the funding round. In addition to refinancing existing debt obligations, this deal provided additional capital to advance AI capabilities and increase the corporate adoption of the company's medical platform.
HealthSnap recently launched an AI-enhanced solution for managing advanced primary care. It integrates agentic AI with clinical workflows, helping medical teams manage large patient groups more efficiently.
The company's platform integrates numerous functions into a single system compatible with electronic health records. These functions include remote patient monitoring, chronic disease management, primary care management, advanced primary care management, corporate analytics, reimbursement optimization, and care coordination.
Chief Executive Officer and co-founder Samson Magid stated that this funding validates HealthSnap's long-term vision: to make continuous and intelligent healthcare the standard in modern medical institutions. He also emphasized that artificial intelligence is intended to augment the work of clinical teams, not replace healthcare professionals.
HealthSnap is demonstrating rapid growth amid the increasing adoption of AI-driven care management among healthcare providers. The platform processes two dimensions of patient data every second while simultaneously reducing the frequency of clinical alerts by 97% for patients after their first year of participation.
Currently, the company serves over 80,000 active patient programs across 200 medical systems and physician organizations. By the end of 2026, HealthSnap expects to support over 100,000 active care programs globally.
Among HealthSnap's business partners are Prisma Health, AdventHealth, Ascension Health, and Sentara Health, located in various parts of the U.S. Partners also include Tampa General Hospital, UnityPoint Health, Baptist Health South Florida, and Mount Sinai Medical Center.
Most aspects of care delivery by HealthSnap can now be supported by artificial intelligence. By 2025, the company's AI platform analyzed over three million clinical progress records, significantly exceeding the capacity of manual audits and contributing to improved service quality.
AI-simulated patient interactions allow newly hired nurses to practice patient engagement. The company notes that this contributes to achieving a Net Promoter Score of 72 while improving employee and patient engagement.
The platform also generates clinical summaries, which help systematize disparate patient information for faster and more informed clinical decision-making. AI agents also assist with patient registration, device support, and protocol compliance queries.
Over the past three years, HealthSnap has increased its revenue by more than fivefold and demonstrates stable annual business growth. The company received the 'MedTech Breakthrough' recognition for 2025 for both clinical workflow and operational competence.
Thanks to successful organic business development, HealthSnap has solidified its position as a fast-growing player in the healthcare market and was included in the list of one of the fastest-growing American companies in the Inc. 5000 ranking for 2025.
