A twelve-year-old boy named Eugene, born with Mobius syndrome, successfully underwent a highly specialized facial reanimation procedure at the Donald Gordon Medical Centre of the University of Witwatersrand (WDGMC). This operation allowed him to smile for the first time and restore facial muscle movement.
Due to a rare neurological disorder that affects facial expression and mimicry, Eugene had been unable to smile, blink normally, or express emotions through facial movements his entire life. The procedure involved transplanting functioning muscles along with their blood vessels and nerves onto the face. Surgeons restored the delicate structures under a microscope, which eventually allowed the movements to return.
The operation was carried out with the support of the Smile Foundation, which works to improve children's access to reconstructive surgery requiring special care. Tari Parfitt, CEO of the Smile Foundation, emphasized that this procedure is more than just a medical intervention. She expressed deep gratitude to the Donald Gordon Medical Centre of the University of Witwatersrand and Dr. Dimitrios Liakos for supporting Eugene. Parfitt noted that providing this level of assistance to a family in a world-class center is an outstanding achievement because facial reanimation fundamentally changes a life. For Eugene, it opens up possibilities for self-expression, communication, and a future he might not have dreamed of.
Dr. Dimitrios Liakos, a plastic and reconstructive surgeon specializing in reconstructive and microsurgery, stated that the goal of the work goes beyond appearance. He explained that such procedures aim to restore function and movement, helping patients regain lost or initially absent parts of life. Reconstructive microsurgery involves transplanting tissues, muscles, and nerves from one part of the body to another, followed by connecting blood vessels under microscopic magnification.
Although similar operations are performed in specialized centers worldwide, access to this level of medical care in South Africa remains limited due to the need for advanced infrastructure, multidisciplinary expertise, and years of specialized training. WDGMC initiated a reconstructive microsurgery apprenticeship program, the first of its kind in the country, to increase the number of specialists capable of performing these complex operations. Liakos stressed the responsibility not only to perform surgeries but also to pass on skills, otherwise, access to such care will remain restricted.
He also noted that the success of reconstructive microsurgery depends on the coordinated work of various medical teams. Liakos explained that microsurgery is never the work of one person; success is ensured by medical staff, anesthesiologists, operating teams, and the entire surrounding system. The doctor added that these operations can last from 10 to 12 hours and are physically and emotionally taxing, but the realization that the work can change a person's life forever gives meaning to every moment spent in the operating room.
